FAQ
I am a provider for Blue Cross/Blue Shield, Midlands Choice, Aetna, Cigna, and Healthsmart. Your coverage and out-of-pocket costs for services are based on your insurance plan. I recommend contacting your insurance company to determine your mental health benefits. This the information you will need when you call your insurance company:
Billing NPI for Loren Conaway, PhD, LLC: 1538865613
Practice Name: Loren P. Conaway, PhD, LLC
Procedure Codes:
90791 (Initial Assessment) - first appointment
90837 (individual Therapy) - subsequent 55 minute sessions
Please be aware that reimbursement is considered a matter between you and your insurance company. It is important to check with your company about coverage and benefits before starting treatment.
I provide out-of-network rates if I am not paneled on your insurance plan. You may have the opportunity for therapy costs to be reimbursed from your HSA, HRA, and/or FSA, or as out-of-network claims to your insurance company depending on your plan (contact your insurance company for details).
Telehealth sessions are covered by most insurance providers. However, I recommend checking with your insurance company to determine your benefits. Telehealth is completed via a HIPAA compliant platform in order to protect your privacy and confidentiality. This technology is provided at no cost to my clients.
I accept credit cards, checks and cash. Balances are to be paid on a monthly basis.
Sessions are typically 55 minutes in length. Missed appointments or cancellations without 24-hour notice will result in a no-show or late cancellation fee.
The first session focuses on information gathering while we become acquainted. I welcome questions! If time permits, we will begin to develop a collaborative treatment plan.
Monday through Thursday and Saturdays 8 am-5 pm
